They don't always know, and in the Marvel universe regular old super heroes don't tend to be universally loved and admired the way DC super heroes are. However, a few rules of thumb about mutants in Marvel and why they get the hate/fear they do (theoretically at any rate).

• Most people consider themselves to be at VERRRYYY long odds to be bitten by a radioactive animal, be a panthestic god, a secret alien, find an alien artifact, stumble into toxic waste, etc, etc. These situations would be considered astronomical bad luck, and (though some people would be bigoted jerks to a "victim" of such things) it's less likely that someone would react as negatively to say "a person who lost limbs in an accident" than a person of color, if you're that kind of small minded person.
• Being a mutant can happen to ANYONE. You don't have to have been taking pictures in a lab, excavating in a forgotten temple, saving an old man from a truck filled with toxic waste...you can just be born...minding your own business, mopping the floor and BAM...you're growing a beak, or have weird dark force energy leaking out of your ears. That's where a lot of the fear comes in. This fear and fear of the other is the basis of the original idea of the Civil Rights struggle in the 1960's that the X Men were meant to be a parable for. People being feared and hated just for how they were born.
• How can you even tell if a hero is a mutant? You can't always. In the past some heroes have been thought to be mutants that weren't (The original Mimic from X Men), and some mutants just didn't let it known that they were, for obvious reasons (in the original Ultimate Universe the Wasp hid that she was a mutant for instance), and there are some that the public simply don't realize are mutants (darn near all of the Great Lakes Avengers).
