I was looking up Polaris's page and it said that Madrox is dead ? ok 1) When did this happen ? and 2) How did it happen ?

These "dupes" can later be reabsorbed, along with all of their memories and experiences. Best known for his X-Factor history, he recently rejoined the newly reformed X-Corporation.
How did Madrox die ?

What do you mean he is not really dead ? I don't think Madrox has a healing factor ?

He wakes up. If you've been reading X-Factor then you know this scene is connected to the vision Rahne had at the beginning of the volume
So it would appear that his conscious leaped forward in time or something. Either way, he isnt truly dead.
