Connor Hawke is in an operating theater after being shot in the head by Onomatopoeia, a serial killer targeting non-powered costume adventurers. In the previous issue, surgery was abruptly interrupted by the killer, but fortunately, Green Arrow, Black Canary, and Mia Dearden were on hand to intervene.

This issue opens with a Mexican stand-off, which leads to Green Arrow placing a nocked arrow in Onomatopoeia's mouth and promising that if Connor dies, so does he. 30 minutes later, one of the doctors who survived the serial killer's initial assault announces Connor will make it. Onomatopoeia then attempts to escape. Ollie evacuates Canary, Mia, and the doctors, and hero and villain engage in a firefight which leads to the roof of the hospital. Already injured from the battle, Onomatopoeia ambushes Green Arrow, who appears to go over the edge of the rooftop. However, Ollie appears and stabs an arrow through Onomatopoeia's foot, sticking him fast to the ledge. After a moment of fighting Onomatopoeia begins to fall off the ledge, being held on by only the arrow through the his foot. He purposely pulls the arrow from his own foot, falling. When Ollie gets to the ground, the killer has escaped. An off-panel search of the area reveals that Onomatopoeia has killed a bystander in a parking garage and stolen his or her car. The issue ends with Ollie and his allies in Connor's hospital room, where he has regained consciousness.
