So when they answer a phone to logout and go back to the real world their physical matrix bodies vanish. However, when the steak guy unplugged the butch lady and the asian dude their physical matrix bodies stayed in the matrix. Theories or thoughts?

I think this was their "avatars" (the representations of their minds) in the Matrix "dying" as their physical bodies disconnected and they died in the "real" world. It would have made just as much sense if they'd vanished in the Matrix when he unplugged them as well, but I suspect they did it this way because it looked more dramatic.

When you jack in, your mind enters the Matrix, and your body is in the real world. When you're unplugged prematurely, your mind is still in the Matrix, which is represented by their "avatars." What you're seeing in the Matrix is their minds dying and, since their minds are still technically in the Matrix, their avatar is still visible.

In the third movie, Neo's mind and body become independent. His physical body remains in the real world while his mind and avatar are conscious in the Matrix. Now, he is the "One" so the rules are different, but it's similar.

@jedixman: It still doesnt make sense man because even the agents need an avatar and cant materialize without one. This implies that neo and company need a physical body in the matrix to act as an avatar. First scene when trinity picked up the phone at the last second her avatar body should have been in the rubble with her mind safely back irl.
