Well, that's easy. Soul Reaper Captain matchups are pretty easy in-verse for Bleach, because all captains have canon stat charts. If we have any doubts about how their feats would compare to one another, we can just compare their stats and this can give us insight into how this would go down:

Unohana battle data (canon):

Attack Potency: 100

Durability: 80

Speed: 70

Hax: 100

Intellect: 100

Physical Strength: 90

540/600

Aizen battle data (canon):

Attack Potency: 100

Durability: 90

Speed: 90

Hax: 100

Intellect: 100

Physical Strength: 80

560/600

Their Attack Potency both physically and with hax (reiatsu + kido abilities) are an even match (100 vs 100 & 100 vs 100). And this much is pretty consistent with their feats. We have Aizen absolutely fodderizing Ichigo and cutting down some of the most powerful Gotei Captains like Kyoraku when they were distracted and worn down, and we have Unohana easily overpowering post Hueco Mundo Kenpachi, that's also on the top of the Gotei power. So they are equals when it comes to their ability to hurt eachother all around.

They also both received 100 for Intellect, but I think Aizen has displayed at least slightly superior intellect when he was able to take down entire Gotei 13 with careful planning and strategy twice in two subsequent story arcs. He also claims to be the smartest guy in Gotei, so we also have his statement on top of his feats. Even though they are close to being equals, I think we should assume it's possible for Aizen to outsmart Unohana as they fight. A slight edge in his favor but it's at least a possibility.

Then Unohana has a slight edge in physical strength. I guess it could be argued that this is consistent with her feats based on her fight against Kenpachi, where she could overpower him in sword clinches at so on, but Aizen has absolutely monstrous strength feats himself, like one shooting Kommamura's bankai or catching Ichigo's sword with his index finger. Still, since their overall offensive capabilities are equal this will only be a very situational advantage for her. It IS an advantage in her corner, just not a major one in the full context.

Sadly, we then have a STUNNING advantage for Aizen when it comes to speed. 20 points of difference is a BIG difference on these charts, and Aizen has MUCH better feats in his corner too (blitzing bankai Ichigo completely while he was staring in his face for example, and easily dodging his masked getsuga tenshou. Unohana didn't do anything impressive tbh). And this is no small advantage either. While stuff like an advantage in strength is situational, speed is basically the key of the battle. Unohana is MUCH slower than Aizen, so Aizen will surely cut her and blitz her more often than he will get tagged himself.

And a final nail to the coffin - Aizen also has an edge in durability. It may not be a big one (10 points) but still. I think this is also demonstrably true based on his feats like surviving Yamamoto's Itto Kasso with nothing but slight burns. So now if they trade similar injuries, Aizen is going to hold up much better too.

At the very least, we should also consider what is not in the charts - their unique swords. While OP gives Unohana a benefit of not being under KS influence from the start, this really gives her no advantage, considering her own Zanpaktou is basically useless. We know absolutely nothing certain about her bankai, and her shikai is only any good for healing and transporting others. The official "guess" on how her bankai probably works is that it produces strong acid that can melt you to death. Well, something like that should be easy to dodge or block with kido, just like any other offensive spell Unohana uses, so it really gives her no new, unique way to fight and to gain an advanage.

Meanwhile Aizen, even though he doesn't have her under his spell yet, is basically forcing her to fight completely blind. Unless Unohana shuts her eyes from the start, there is no way she can avoid at least briefly looking at Aizen's sword, and once she does, it's GG.

To sum up, what Unohana's sword on paper gives her, is a new acid-like offensive spell to attack Aizen with, while what Aizen's sword gives him on paper, is either a completely and permanently blinded enemy, or an instant victory. I'd say Aizen's sword gives him an objectively much superior advantage then.

Aizen wins.

While in some areas they are equals, Aizen is way too fast for her both by stats and by feats, more durable by both stats and feats, a tad smarter by feats, and has a better zanpaktou ability, that even with the OP nerf will be much more useful than Unohana's sword. On top of all that, the only meaningful advantage on Unohana's end is a slight edge in strictly physical muscle strength, that is not even supported with her feats for the most part.

Unohana has a chance here. She was keeping up with a version of Kenpachi that was waaaay stronger than the Kenpachi that Aizen wasn't sure about fighting in FKT. Her bankai would also make quick work of Aizen should he be caught in it. Unfortunately, that's all I can say about her.

Even if she wasn't under KS at the start of the battle, Aizen would simply put her under it straight away as it is within his character to do so. Unohana's shikai and bankai need time to properly activate their abilities; time that aizen can use to blitz her as she prepares. HOWEVER, if she gets her bankai off, then she can keep aizen at bay until she catches him with the very potent acid.

Imma give it to Aizen 7/10 times.
