The seals that Hagoromo gave them was to seal Kaguya and that was because he couldn't do it himself (not that he is weaker than Kaguya its because he can't interfere with the real world for long), Hagoromo knew Naruto and Sasuke couldn't win at that point so he gave them the ability to do the same thing he and his brother, Hamura, did.
After they sealed her the seals disappeared but Naruto still showed the TSB and Sasuke still had the Tomoe Sharingan in their fight. The fact that after losing the seals and the respective hands they were on, Naruto and Sasuke where still able to free all the captives of the IT showing they still had Hagoromo's chakra.
Naruto's SOSP appears as him having the Fox slits and Frog slits without pigmentation around the eye
He then mixes it with his Kurama transformation
You get this form by being
1) The ten tails jinchuriki (Hagoromo, Obito, Madara)
2) The jinchuriki of all the tailed beasts (Naruto)
They look different because of the ways you go about getting it and Naruto has lost the TSB (no reason for that maybe he can regen them but that's speculation) but he can still interact with them just fine
And as an adult Naruto has gotten the other half of Kurama meaning he has little pieces of all tailed beasts chakra and full Kurama and Sasuke has now gotten used to his eye and its powers, so they got stronger during the blank period.

IDK how many times I've said this, the seal is not JUST a seal, it also hosts Six Paths Power which is the source of Hagoromo's Chakra for Naruto as confirmed when we saw through Kaguya's Byakugan.
Yes after the seal is gone he still have TSB etc but the residual chakra already produced is still present, even after the source of it is gone.
After VOTE2 battle where both of them exhausted every last drop of chakra within them, Naruto no longer possess any more of Hagoromo's Chakra, and not being able to replenish it either due to losing the source. (the story is slightly different for Sasuke because he possess the actual Indra+Asura DNA)
This is proven in The Last movie, when he could only use up to BSM as his most powerful form. Also, Toneri could absorb all of Naruto's chakra, but not Hinata's, due to Hinata possessing Hamura's Chakra. If Naruto possess Hagoromo's Chakra he would've had the same resistance.
Now the reason why Naruto can once again use SPSM as Adult is because he gained another source of Six Paths Chakra that's not the seal. That is by being the pseudo Ten-Tails Jinchuriki, by possessing all of Tailed Beasts' Chakra.
This is why you see Adult Naruto able to go Six Paths ONLY when the Kyuubi Mode is also active. In base, he has always used regular toad Sage Mode, even though SPSM is much more effective.
However, even as a teen, this source of Six Paths Chakra is quite insignificant compared to the seal, which is the main source of Six Paths Chakra for Naruto. This is what allows Naruto, even with just a shred of Tailed Beasts Chakra, to compete with Madara with the FULL chakra of all the Tailed Beasts.
