If he kills Azatoth he will die too as he is part of Azatoth's dream.

Gilgamesh isn't bound to a single reality so if the dream ends, he'll survive. This is supported by the ending of Fate Extra CCC where he left the moon cell — The simulation that had generated his existence and had sustained it — and went to some random galaxy that existed in some higher level reality than the moon cell with ease, he wasn't even strained. Therefore, Azazoth's dream wouldn't apply to him; it would just be an ordinary dream as any other is in his eyes.
