it's interesting how difficult this question is to answer. you could say a number of villains REALLY hate him, but they have also shown moments of mercy towards spider-man or actually liking him. for example, you could say it's osborn, but osborn also at one point decided he wanted peter to be his son. you could say it's doc ock, but doc ock has definitely shown peter mercy at least a couple of times.
and the villains that aren't in this situation, i don't think feel as strongly about their hatred of spider-man. so it's a tough question.
if we're talking about 616 spidey here, i might say morlun, because he seemed to hate 616 spidey more than all the other spider people, and i think that's saying something.
or (and this is another cop-out) the person who hates spider-man the most might be himself?

norman and the symbiote are wrong answers .. norman loves peter as a son more than he loves harry because he considered harry to be weak and useless .. norman has had more than a hundred opportunities to kill spiderman ..
i would have to say hobgoblin or scorpion that is why scorpion is stuck in that thick hide because of spiderman and hobgoblin ( jason phillip macendale) really hates spiderman he said he wanted to eat spidermans dead body after he killed him and spider says " dont you think that is a big over doing it " hobby says " absolutely not ,you have no idea how much pain you caused me "
.. when kingpin ( richard fisk) goons had spiderman in shackles hobgoblin was bitching the whole time that we should just kill spiderman now , kill spiderman now! while we have the chance!
and to top it off hobby had to merge with a demon in order to get stronger so he could get physical with spiderman and then he had to try to separate himself from the demon .. kind of screwed up his life dont you think?

peter didnt cause gwens death i think your thinking of the movie version... peter has saved gwen mutiple times from villians that didnt even know peter was spiderman

The One of Above All(Marvel U's God). Basically he has created a law out of the whole multiverse that must define his very existence which has designated Peter to being the fundamental abstract concept of suffering and bad luck.
